During Sleep endoscopy one can see a large floppy epiglottis closing like a door over the laryngeal inlet. In this video one can see a door like epiglottis literally blows down and completely occlude the laryngeal inlet. The patient complained of suffocation while using CPAP. The somnoendoscopy was performed prior to multilevel OSA surgery , a laser epiglottecomy is advised at a later date with tracheotomy consent.

Moral-Don’t forget the Epiglottis in failed OSA surgery or CPAP therapy..
