Foreign Nationals Coming For ENT Treatment in Jubilee Hospital – IMPORTANT

1-For Obtaining medical treatment Visa to come , I am not allowed to give any recomendation or invitation letter to have treatment in Jubilee hospital, without me seeing the patient.

2.For any treatment in the hospital, the patient MUST be accompanied by a close relative or friend.

3.All payment in the hospital must be in Indian Rs only, no debit credit cards will be accepted.

4.Please bring a letter of referral from registered doctor of your country with details.

5-On arrival at the reception for taking appointment ,The Hospital Authority needs to collect the copy of passport, the visa page, emigration stamp ?(Entry) page of the patient and the attendant.

The bill/phone number or booking receipt of the Hotel the patient staying before admission.

6.Collect FORM -C from the foreign cell of the Police commissioners office at vazhuthacadu.

7.Complete the Form C. Ask the patient to sign the Form C.

8.Take the completed Form C and above documents to Foreign Cell of the Police commissioners office in Vazhuthacadu ( Behind the main building) and copy of the same to the nearest police station- Museum police station.

9.In Form C, Please mention the particular treatment not available in their country and they came for specialized treatment here.

10.Once the patient is admitted, both the patient and bystander must not leave the ward/hospital without permission. It is better not to allow to leave the ward at any cost out of hospital premises.

11.Once the treatment is completed, its the duty of the treating doctor and Hospital to inform by a written note and copy of the discharge summary to Foreign cell of the commissioner office.
