What is Rhinoplasty?
Also known as Nose Job, Cosmetic Surgery of Nose, improves the appearance of the nose and face which in turn enhance the facial harmony and self confidence.
Nose, Being the most projected part of face, is noticed first when one sees our face. Every Nose is unique, so too every Rhinoplasty procedure.
What are the Possibilities in Rhinoplasty?
Cosmetic Surgery of Nose or nose job can change:
and Nose size, in relation to other facial structures
and Nose width at the bridge or in the size and position of the nostrils like Broad Nose, Hump Nose or Bumpy Nose, Big or Bulky Nose, Small Nose or Low Dorsum or Bridge of Nose
and Nose profile- visible humps or depressions on the bridge; Long Nose with Drooping tip, Lateral Deviation of Nose or Crooked nose
and Nasal tip- large or bulbous, drooping, or too upturned
and Nostrils- that are large, wide or upturned
and Nasal asymmetry and deviation
and Deviated Nasal Septum (DNS)/Septal deformity
and Revision Rhinoplasty
Limitations: Unrealistic Expectations
Since nose is your most prominent facial feature, even a slight alteration can greatly improve appearance. Before surgery, it is very important that the patient have a clear, realistic understanding of what change is possible as well as the limitations and risks of the procedure.
ENT or Plastic Surgeon to choose?
Surgery of the nose may also correct nose block or other sinus problems caused by structural abnormalities in the nose in the same operation. ENT surgeons are trained and experienced in treating all internal conditions of nose. That\’s why always an ENT surgeon with plastic surgical experience does better nose job than plastic surgeon.
How it is done?
1. External (Open) Rhinoplasty: an incision is made across the columella, the narrow strip of tissue that separates the nostrils, made in the shape of an inverted V, a W, or a zigzag, which is referred to as a “step off” incision. Through these incisions, the soft tissues that cover the nose are gently raised, allowing access to reshape the structure of the nose.
2. Closed Rhinoplasty: Doing the procedure thru the nostrils. Here the procedure is performed thru a closed Rhinoplasty technique, where surgery will begin with the surgeon making small incisions just inside the rim of each nostril.
Anesthesia
General Anesthesia is preferred in most cases. Small defects can be corrected by local anesthesia.
Reshaping the nose structure
Depending on the characteristics of your nose, your Rhinoplasty may involve reshaping one, some, or all of the following: the bridge of your nose, the upper portion of your nose, the cartilage at the tip of your nose, and the nostrils.
Functional Septorhinoplasty
If it is necessary to improve the function of your nose as well as its appearance, we may perform additional reshaping of the septum and other tissues within the nose. If the septum is deviated, it is now straightened and the projections inside the nose are reduced to improve breathing.
Augmentation Rhinoplasty
Surgery of the nose can reduce or augment nasal structures with the use of cartilage grafted from other areas of your body. Most commonly, pieces of cartilage from the septum, the partition in the middle of the nose, is used for this purpose. Occasionally a piece of cartilage from the ear and rarely a section of rib cartilage can be used.
Reduction Rhinoplasty
Surgery to reduce the dorsal hump, bulbous tip to get a smaller size nose.
Closing incisions
Once the best results possible have been achieved, ENT surgeon will close the incisions. The incisions inside the nose and across the columella are closed using very fine sutures that are about as thick as a single hair (5×0 Ethilon or Prolene). The sutures used in Rhinoplasty may be absorbable, non absorbable, or a combination of both. Absorbable sutures dissolve on its own and don’t require removal. Non absorbable sutures require removal later in our outpatient department.
Postoperative care
After your procedure is completed, a splint, Merocel packing will likely be placed inside your nose and a splint or bandages placed on the outside of the nose to support and protect the new shape during initial healing.
You will be given antibiotics and anti-inflammatory medications for 5-7 days.
Splints and internal tubes will likely support the nose as it begins to heal for approximately one week.
How long it will take to get Final result?
While initial swelling subsides within a few weeks, it may take up to a year for your new nasal contour to fully refine. During this time you may notice gradual changes in the appearance of your nose as it refines to a more permanent outcome. Swelling may come and go and worsen in the morning during the first year following your nose surgery. It may take several months for swelling to fully dissipate and up to a year – and sometimes longer – for the outcome of the surgery to fully refine.
