Surgical Treatment available for the treatment of snoring and OSA
A number of surgical procedures are available to treat Snoring and OSA. Effective surgical management of OSA depends upon determining different levels of obstruction, which may include nasal, nasopharyngeal, oropharyngeal, and hypopharyngeal, retrolingual, or a combination of these sites.
Surgical treatment may involve various procedures that are performed in different stages depending on the patient\’s sites of obstruction. The most commonly performed procedures include nasal surgery-Septal Correction, Nasal polyp removal, Turbinoplasty, uvulopalatopharyngoplasty (UPPP), advancement genioplasty, mandibular osteotomy with genioglossus advancement, and hyoid suspension. In more severe cases, maxillomandibular advancement (MMA) with advancement genioplasty may be indicated. As a last resort Tracheostomy is considered.

Laser Assisted Uvulopalatopharyngoplasty (LAUP)
Laser uvulopalatopharyngoplasty (LAUP) is commonly done for the management of obstructive sleep apnea along with other surgical procedures to remove the cause of OSA in the upper airway in Jubilee Hospital, Trivandrum, Kerala, South India. Nearly one hundred cases are performed in last one year.
Patients are given a choice of CPAP therapy or surgery. Most patients find it difficult to live on CPAP. They say they don\’t want to become slaves to CPAP.
Indications for surgical treatment include an elevated respiratory disturbance index (RDI) with excessive daytime somnolence (EDS), oxygen desaturation below 90 per cent, medical complications including hypertension and arrhythmias, anatomic abnormalities of the upper airway and failure of medical treatment. The success of surgery in OSA is generally measured by achieving a (RDI) of less than 5, improvement of oxygen concentration to 90 per cent or more with no desaturation below 90 per cent and quality of life improvements with elimination or significant reduction of OSA symptoms.
